 Job Description
CentacareCQ has an opportunity for a part-time NDIS Plan Officer to join their dedicated Health & Wellbeing Services Team, based in Mackay (45.6 hrs per fortnight).

The NDIS Plan Officer will be responsible for the provision of information, support and account management services to NDIS clients. This will include working with other agencies, family members and stakeholders to ensure clients needs, preferences and aspirations are met.

Your duties will include:

Main Duties

* Provide appropriate intake, support, information and referral of eligible clients;

* Review client needs to ensure appropriate and efficient service delivery and referral including the development of care plans as needed;

* Ensure all client records are up-to-date, with up-to-date assessments, budgets and care plans relating to individual client goals and choices;

* Ensure data entry is completed as per funding agreements and CentacareCQ policies;

* Participate in meetings as required by the role.

Continuous Improvement

* Ensure programs follow a continuous improvement model of service delivery, including participation in development and review of procedures and work instructions for the service in consultation with the Health & Wellbeing Services Manager and Quality Team;

* Ensure all documentation is completed to meet relevant standards and audit requirements.

Community and Promotions

* Undertake community engagement to promote CentacareCQprograms and increase numbers of clients engaging with CentacareCQ under the NDIS Program;

* Develop and maintain networks in the community with other community agencies and stakeholders in consultation with the Health & Wellbeing Services Manager.

Please note: The successful candidate will be required to participate in an Australian Federal Police check and a Working with children check, prior to confirmation of appointment to the position.The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.
