CentacareCQ has an opportunity for a fulltime Case Manager Community Care Services to join their dedicated Community Care Team, based in Rockhampton (60hrs per fortnight).

As part of CentacareCQ's Community Care Team, this position works under general direction in the application of procedures, methods and guidelines which are well established. It involves employees solving problems of limited difficulty using knowledge, judgment and work organisational skills acquired through qualifications and/or previous work experience. Assistance is available from senior employees.

Key duties include:

Clients and Services

* Process and conduct client intakes, referrals and assessments, and reviews as per program guidelines and protocols in consultation with the Senior Coordinator Aged Care Services;

* Liaise with necessary stakeholders regarding ongoing client care;

* Develop comprehensive individualized personal care plans and goals for each client in collaboration with the client and their family that incorporates Consumer Directed Care practices;

* Provide services in line with the Quality of Care Principles 2014, Commonwealth Home Support Program 2017 and CDC practices ;

Organisation Policy

* Appropriate use of Organisation resources: HR3, iCareHealth, vehicle booking system, RiskMan, Workplace and other CentacareCQequipment

Documentation and Communication:

* Complete a range of timely documentation in line with CentacareCQ's policies and procedures including client service agreements, budget tools and care plans relating to individual client goals, needs and choice; case notes, safety checks, consent forms, updating registers, client reviews;

* Maintain client files, program registers and undertake client file audits as required.

Continuous Improvement

* Assist Senior Case Manager with identifying and implementing continuous improvement activities;

* Assist Senior Case Manager with internal and external program audits;

* Attend and participate in internal and external meetings as required.

Please note: The successful candidate will be required to participate in an Australian Federal Police check and a Working with children check, prior to confirmation of appointment to the position.The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.
