 Job Description
CentacareCQ has an opportunity for a full-time NDIS Plan Facilitator to join their dedicated Health and Wellbeing Services Team, based in Rockhampton.

The NDIS Plan Facilitator will be responsible for the provision of information, support and account management services to NDIS clients. This will include working with other agencies, family members and stakeholders to ensure clients needs, preferences and aspirations are met.

In partnership with the NDIS Business Development and Support staff member, the primary responsibility of the NDIS Plan Facilitator will be to represent the organisation within the local community, to promote services provided by CentacareCQ.

Duties of the role include:

* Main Duties

* Providing appropriate intake, support, information and referral of eligible clients

* Planning, coordinating, implementing and administering the activities of the program, in consultation with relevant CentacareCQ teams (e.g. roster team)

* Reviewing client needs to ensure appropriate and efficient service delivery and referral, including the development of care plans and behaviour support plans as needed

* Ensuring all client records are up-to-date, with up-to-date assessments, budgets and care plans relating to individual client goals and choices

* Ensuring data entry is completed, as per funding agreements and CentacareCQrequirements

* Monitoring and interpreting legislation, regulation, program standards and other agreements relating to program delivery, staff and clients, in consultation with the Health & Wellbeing Services Manager

* Participating in meetings as required by the role, including Diocesan working groups, as designated by Diocesan Management.

* Identifying skills and workforce development opportunities required by staff to deliver services to clients under the NDIS, and making recommendations to the Health & Wellbeing Services Manager

* Building positive connections with the broader CentacareCQteams

* Continuous Improvement

* Ensuring programs follow a continuous improvement model of service delivery, including participation in development and review of procedures and work instructions

* Assisting with internal and external program audits

* Community and Promotions

* Undertaking community engagement to promote CentacareCQprograms and increasing the number of clients under the NDIS Program

* Developing and maintaining networks with other community agencies and stakeholders

Please note: The successful candidate will be required to participate in an Australian Federal Police check and a Working with children check, prior to confirmation of appointment to the position.The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.
